Aperion Care Bloomington, a 117-bed skilled care facility in Bloomington, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the fourth quarter of 2017.
The report lists the nursing homes in the state with AA or A violations, which might have caused the death of a resident or could cause death or serious injury to a resident, respectively.
Aperion Care Bloomington was cited with an A violation in October and fined $25,000, and was fined $2,200 for a B violation. The facility was also cited with an A violation in November and fined $25,000, and fined $4,400 for two B violations.
A B violation indicates a nursing home incident or situation that is unlikely to cause more than minimal physical or mental harm to a resident.
